Info for New Patients

1. Get a referral/ prescription from your doctor

Get a referral/ prescription from your doctor. Your doctor can either give the form to you to bring with you on your first visit, or your doctor can fax it to our office at 203-776-4989. Although Connecticut State Law no longer requires you to have a physicians referral, insurance companies still require a referring physician to be listed on claims.

Auto insurance accident patients

If you’ve had an auto accident, please have the following information available:

  • The name of your auto insurance company.
  • Whether or not you have medical coverage under your auto policy and the amount. If you don’t, please get a letter from your auto insurance company to our office by the time of your first appointment. Insurance companies call this a “no med pay” letter.
  • The name and phone number of your accident insurance adjustor
  • Claim number
  • The date of the accident
  • The name and phone number of your attorney

Worker’s compensation injury patients

If you have a workers compensation injury, please provide us with the following information:

  • The name of your worker’s compensation insurance
  • Claim number
  • The name and phone number of your adjustor
  • Date of the injury
  • Name, address, and phone number of where you worked when the accident  occurred
